Emergency Medical Care:

If, in our judgment, your pet(s) requires immediate medical care for illness or injury or if your pet(s) has a pre-existing condition which is aggravated by its stay and it requires professional attention, and we are unable to reach you or your emergency contact we, Rainbow Paws Boarding Kennels, at our sole discretion, will take your pet(s) to the nearest veterinarian or animal hospital.

□ Yes Initials ____ By initialing here, you agree to be solely responsible for the payment of all medical bills for your pet(s) and you release Rainbow Paws Boarding Kennels, it’s owner’s, director’s, and employees of and from any and all responsibility for, or claims, damages, debts, arising out of or related to such medical care, including, but not limited to, transportation to/from veterinary clinic and choice of veterinarian and animal hospital.

Please read carefully and initial each item:

___ 1. Owner specifically represents that he/she is the owner of the kenneled pet(s), or has been authorized by the owner of the pet(s) to enter into this Contract as the Owner’s Agent.

___ 2. Owner agrees to pay the boarding rate for all services owed prior to checkout. Owner is aware that the kennel only accepts payment by cash or credit card by Square (transaction fee applies). Rates are subject to change.

___ 3. Owner understands and agrees that pet(s) shall not leave the Kennel until all charges are paid. Owner is also aware that if the date and/or time of pick up is extended without prior approval of Kennel there will be an additional charge.

___ 4. Owner understands that there is a nightly boarding fee charged on day of arrival (regardless of drop off time), and there is a full day charge for the day of departure, except for pick-ups before 10 am.

___ 5. Owner understands that extra charges will be added to bill for pick up outside of the regular pick up hours after 6:30 pm. Kennel must approve said action in advance of boarding and will advise owner of the fee amount & terms at the time the request is approved.

___ 6. Extra Charges may be added to the bill at the discretion of the kennel owner for special handling or treatment that is above and beyond routine care, i.e. an unexpected bath, etc. Special handling is defined as services or treatments beyond the standard boarding care due to behavior problems, excessive accidents, health issues, or other unexpected care not anticipated at time of drop of. Owner agrees to pay all such charges.

___ 7. A No Show Fee will be charged if pet(s) are not dropped off or picked up at the specified time & date agreed upon. If the entire Boarding Reservation is a No Show, Owner will be billed 50% of the Boarding Reservation. If you need to cancel your boarding, please do so at least three (3) days prior to your arrival date. Rainbow Paws Boarding Kennels reserves the right to charge a late cancellation fee.

___ 8. If Owner is running late, then he/she is aware that the Kennel has a policy of waiting no more than 30 minutes for free without exceptions. He/she should also be aware that excessive late pick-ups or drop-offs can result in no longer being able to board at Kennel.

___ 9. Kennel owner and/or helpers are not veterinarians or registered veterinary technicians and does not board pets with medical conditions other than those stable with oral medication.

___10. Kennel is only moderately supervised and is not liable for pets that are jumpers/climbers, diggers or others that can escape dog run enclosures, have behavior problems, allergies i.e. bees, skin, etc., eats gravel/rocks, are unstable on uneven surfaces such as gravel/rock, encounters dogs shedding parasites (worms), and other such occurrences that can occur in an outdoor setting.

___11. If Kennel has to supply food, there will be an additional charge per day per pet. Owner should be aware that a change in food may result in diarrhea. In the rare event of diarrhea, owner is aware that the Kennel uses Pepto-Bismol or Pumpkin to help provide relief. Both are safe for animals and recommended by Veterinarians.

___12. All pet(s) entering Kennel must be flea free. If upon inspection, this is not the case, a flea bath, powder or spray may be given or used at Owner’s expense. If your pet(s) has any allergies to any flea products please acknowledge this on the allergy section of this agreement.

___13. Very old pets may be sensitive to a change in their environment and even with personal attention and care that is given to all pets, the change may be enough to encourage an early passing. In this unfortunate event, we will notify you (owner) or your emergency contact immediately.

___14. Kennel shall exercise due and reasonable care for the pet(s) while boarding. Under this care, Owner releases Kennel from, and waives all claims and liability against Kennel for or attributable to, injury or illness of pet. Owner agrees to be solely responsible for any and all acts and behavior of their pet(s) while in the care of Kennel. He/she will be charged Damage Fees for any damage caused by pet to kennel structure and/or Kennel property.

___15. Owner is permitted to bring any bedding, toys, etc, but agrees to release Kennel from, and waives all claims and liability against Kennel for, damage to, or loss of, personal equipment or belongings provided by Owner for pet(s) while the pet(s) is at Kennel. Items not taken home at pick-up will be donated at the discretion of the Kennel Owner if not picked up within 60 days of pick-up.

___16. Female pets in season may not be boarded at Kennel. If your pet(s) show signs of coming in to their heat cycle, an attempt to contact you and/or your emergency contact will be made to pick-up your pet(s). If we are unable to get in contact with you, Owner agrees, pet(s) will be housed in other accommodations away from the main Kennel area until owner or emergency contact is contacted and arrangements can be made for pick-up.

___17. Owner represents that pet(s) are healthy and have not been exposed to any known communicable diseases within the thirty-day (30) period immediately prior to boarding. He/she also agrees to disclose to Kennel all known medical conditions and/or behavior problems which may affect the pet(s) care prior to drop-off and inform Kennel of any changes in pet(s) condition and behavior at or prior to drop-off for all future boarding stays.

___18. Kennel specifically requires that all pet(s) be vaccinated against communicable diseases prior to boarding. We reserve the right to refuse admittance to any pet that shows signs of illness or that does not meet Kennel Vaccination requirements. Despite these precautions, Owner acknowledges that his/her pet(s) will be in an environment with other pets during boarding, and understands that any pet may harbor and spread a communicable disease.

___19. Owner releases Kennel from, and waives all claims and liability against Kennel for, all losses, damages, costs and expenses arising out of or in connection with any communicable disease contracted by Owner’s pet(s) during their boarding.

___20. In an emergency or natural disaster, every effort will be made to contact you(owner) or your Emergency Contact to pick-up your pet(s). You(owner) agree that Rainbow Paws Boarding Kennels, at its sole discretion, is authorized to transport, and/or to make temporary alternative arrangements to house and care for your pet(s) until you(owner) or your emergeny contact can pick-up your pet(s). You also understand that it may not always be possible to safely evacuate your pet(s).

___21. Owner agrees that if pet(s) becomes aggressive, destructive or disruptive, or acts in any manner that puts Rainbow Paws Boarding Kennels, owner, family, other pet(s) in any danger, we will attempt to notify you(owner) or your emergency contact to make other arrangements for your pet(s).

___22. Owner acknowledges that pet(s) that are kenneled together are from the same household, have spent time together, etc. Owner assumes all responsibility for any aggressive behavior, fighting or injury that one pet may inflict upon another while they are together. Rainbow Paws Boarding Kennels reserves the right at our discretion to separate any pet(s) who are boarding together for the pet(s) own safety. If your pet(s) are not compatible boarding in the same kennel unit, you will be billed and responsible for the cost of more than one kennel. If another kennel isn’t available, you or your emergency contact will be contacted for other arrangements.

___23. Owner understands that while your pet(s) are staying, he/she will come into close proximity of other pet(s). Every effort will be made to ensure the safety of our pet guests. Owner, acknowledges that, in the unlikely event your pet(s) is injured by another pet(s), you release Rainbow Paws Boarding Kennels from any liability for such injury. If your pet(s) injures another pet, you will be solely responsible for those injuries and you release Rainbow Paws Boarding Kennels from any and all liabilities.

___24. Owner agrees to allow Rainbow Paws Boarding Kennels , owner, helpers, etc. to use his/her pet(s) name and any images or likeness of his or her pet(s) taken while staying with us, in any form or format, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional materials.

___25. Owner understands that if pet(s) are not picked up with 15 calendar days after the day pet(s) were scheduled for pick-up, or Owner refuses to pay agreed upon charges, pet(s) shall be deemed abandoned and put up for adoption, taken to local animal shelter or pound.

___26. If you need to cancel your boarding, please do so at least three (3) days prior to your arrival date. Rainbow Paws Boarding Kennels reserves the right to charge a late cancellation fee.

___27. Owner understands that Kennel reserves the right to refuse service due to pet’s behavior or health, excessive cancellations, no shows, or late arrivals, non-payment, or violations of said contract.
